Affidavit for Correction of a Birth or Death Record IMPORTANT INFORMATION REGARDING NAME CHANGE: Until the age of 11, the department of Health and Senior Services will accept a ‘Correction Affidavit’ to change the first and/or middle name. Requires that no previous amendments have been done. To request the proper form you may call their office at 573-751-6378 and they will mail you the form that needs to be completed as it is not available online. After completing the form it must be notarized and returned to them. Their processing time for this by mail is 3-4 weeks. If desired, the form can be returned by priority/overnight mail and the processing time is reduced to 7-10 days. Request the physical address for the office if using this option, so that the overnight mail goes directly to them.

The World Professional Association for Transgender Health (WPATH), formerly known as the (Harry Benjamin International Gender Dysphoria Association (HBIGDA), is a 501(c)(3) non-profit, interdisciplinary professional and educational organization devoted to transgender health. Our professional, supporting, and student members engage in clinical and academic research to develop evidence-based medicine and strive to promote a high quality of care for transsexual, transgender, and gender-nonconforming individuals internationally. We are funded primarily through the support of our membership, and through donations and grants sponsored by non-commercial sources. Mission: To promote evidence based care, education, research, public policy, and respect in transgender health. Vision: We envision a world wherein people of all gender identities and gender expressions have access to evidence-based healthcare, social services, justice and equality. Goals and Tasks As an international interdisciplinary, professional organization, the World Professional Association for Transgender Health (WPATH) will work to further the understanding and treatment of gender dysphoria by professionals in medicine, psychology, law, social work, counseling, psychotherapy, family studies, sociology, anthropology, sexology, speech and voice therapy, and other related fields. WPATH provides opportunities for professionals from various sub-specialties to communicate with each other in the context of research and treatment of gender dysphoria including sponsoring biennial scientific symposia. WPATH publishes the Standards of Care and Ethical Guidelines, which articulate a professional consensus about the psychiatric, psychological, medical, and surgical management of gender dysphoria and help professionals understand the parameters within which they may offer assistance to those with these conditions.

The Crime Victim Center of St. Louis (CVC) was founded in 1972 as the first victim service agency in the nation. Our mission is to empower individuals impacted by crime to move from crisis to resilience. We offer a wide range of services for individuals, communities, and other organizations and institutions seeking to improve their services to victims of crime. We provide resources, counseling, advocacy, legal assistance, and referrals for thousands of victims of crime and their families in the St. Louis Metropolitan Area. Services are free of charge and the crime does not have to have been reported to law enforcement to receive services. CVC has different advocacy programs to provide direct services to victims and their families. Our advocates assist victims of intimate partner violence, homicide, sexual assault, nonfatal gun violence, and families/juveniles in crisis. Our counselors provide trauma-specific interventions including person-centered therapy, complex grief counseling, Cognitive Behavioral Therapy, Cognitive Processing Therapy, Eye Movement Desensitization and Reprocessing, Play Therapy, and Hypnotherapy. We see ages 6 and older.
