Affidavit for Correction of a Birth or Death Record

IMPORTANT INFORMATION REGARDING NAME CHANGE: Until the age of 11, the department of Health and Senior Services will accept a ‘Correction Affidavit’ to change the first and/or middle name. Requires that no previous amendments have been done. To request the proper form you may call their office at 573-751-6378 and they will mail you the form that needs to be completed as it is not available online. After completing the form it must be notarized and returned to them. Their processing time for this by mail is 3-4 weeks. If desired, the form can be returned by priority/overnight mail and the processing time is reduced to 7-10 days. Request the physical address for the office if using this option, so that the overnight mail goes directly to them.
