Housing Liaison Program
What We Do
- Outreach to individuals in housing crisis or experiencing homelessness
- Refer and transport (as needed) individuals to initial appointments to establish services for employment, housing, behavioral health, substance use, or physical health.
- Directly contact potential landlords establishing relationships to make referrals for unit occupancy.
- Assisting in applying for Social Security Income, Medicaid, or housing vouchers.
- Help individuals experiencing homelessness meet basic needs.
Who We Serve
- The Housing Liaison outreaches individuals who are not currently engaged in Mental Health or Substance Use Disorder services and are experiencing housing crisis or homelessness.
Accepts Referrals for individuals who:
- Are in and are interested in staying in the St. Louis City area.
- Needing but not currently receiving Mental Health or Substance Use Disorder services.
- Having housing needs.
The Outreach Housing Liaison Program
- The Housing Liaison program is a new grant funded program beginning across the state of Missouri in Sept 2021 aimed at closing the gap between homeless individuals and recourses
- The Housing Liaison position is a critical outreach and engagement facilitator who assists participants in meeting their basic needs, thereby creating opportunities for further treatment and support of other behavioral healtha nd substance use goals
About Hopewell Center
Hopewell Center grew out of a 30-year history of service. ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional metnal health staff serves a growing caseload of 2,800 patients, of which >95% are chronically mentally ill and suffering from major psychiatric disorders.
